40 CFR, Part 71 (i.e., Title V) Initial Permit Application
Contains detailed information on how to complete all of the Part 71 forms.
This form is required to be completed by all applicants as part of an application for a 40 CFR Part 71 Federal Operating Permit.
This form is used to describe emissions units that combust solid or liquid fuels, such as boilers, steam generators, electric generators and the like.
This form is used to describe emissions units that use, process, store or produce substances containing VOC and that primarily emit VOC, such as painting or coating operations and printers. In addition, this form may also be useful for certain HAP emitting sources. The purpose of this form is to help you collect and organize technical data, including operational characteristics, applicable requirements, compliance terms, and emissions.
This form is designed to describe emissions units for processes for which forms EUD-1 or EUD-2 are not appropriate. For example, sources such as rock crushers and asphalt batch plants. This form will help you to collect and organize technical information, including operational characteristics, applicable requirements, compliance terms, and emissions for each emissions unit.
Use this form only if there are any equipment, emissions units, or emitting activities at the facility that qualify for insignificant treatment due to insignificant emissions levels (defined in the part 71 rule) and such treatment is desired.
Use this form to quantify emissions for each significant emissions unit identified in section I of form GIS. This form aids in the organization emissions data needed on forms PTE and FEE. Do not complete this form for any units or activities listed as insignificant on form IE. Sources applying for permit revisions only need complete this form for each emissions unit affected by the change.
Calculates the total PTE for each air pollutant at the facility for purposes of determining major source applicability.
Use this form to initially or annually calculate fees.
Complete this form each time a form FEE is completed and send this form to the appropriate lockbox bank address, along with full payment. This form required at time of initial fee payment, and thereafter, when paying annual fees.
Used to identify current compliance and noncompliance as well as how compliance will be achieved.
This form must be completed, signed by the "Responsible Official" designated for the facility or emission unit, and sent with each submission of documents (i.e., application forms, updates to applications, reports, or any information required by a part 71 permit).
40 CFR, Part 71 (i.e., Title V) Forms to Use Post-Permit Issuance
Use this form to submit written notice to the part 71 permitting authority of each deviation from permit terms that is required to be reported within 10 working days of its occurrence (in other words, deviations required to be reported prior to the 6-month monitoring report). Each form (or multiple forms if sent at the same time) must be certified as to truth, accuracy, and completeness by a responsible official (using CTAC).
Summarizes all monitoring required during the reporting period and provide information on any separate monitoring reports submitted at this time or any time during the reporting period.
Use this form to initially or annually calculate fees. Even if the part 71 program in your area has been delegated to a State, tribe or other entity, you will usually still have to complete this form and pay fees to EPA.
Describe the compliance status of each permit term for the reporting period and report all deviations from permit terms (whether reported previously or not) that occurred during the permit term.
This form must be completed, signed by the "Responsible Official" designated for the facility or emission unit, and sent with each submission of documents (i.e., application forms, updates to applications, reports, or any information required by a part 71 permit).
40 CFR, Part 80 Forms (CFCs and Halons)
EPA requires that persons servicing, disposing, or recycling air-conditioning (other than motor vehicle air conditioning) and refrigeration equipment certify to the appropriate EPA Regional Office that they have acquired (built, bought, or leased) refrigerant recovery or recycling equipment and that they are complying with the applicable requirements of this rule. This certification must be signed by the owner of the equipment or another responsible officer and sent to the appropriate EPA Regional Office. Although owners of recycling and recovery equipment are required to list the number of trucks based at their shops, they do not need to have a piece of recycling or recovery equipment for every truck. Owners do not have to send in a new form each time they add recycling or recovery equipment to their inventory.

Chemical Management
  • Form R, TRI

    1. Description: Toxic Chemical Release Inventory (i.e., Toxics Release Inventory (TRI)) is required by section 313 of the Emergency Planning and Community Right-to-Know Act (EPCRA, or Title III of the Superfund Amendments and Reauthorization Act of 1986), Public Law 99-499. The information contained in the Form R constitutes a "report," and the submission of a report to the appropriate authorities constitutes "reporting."
    2. Who must complete? Owners and operator of facilities that meet all of the following criteria.
      • The facility has 10 or more full-time employee equivalents (i.e., a total of 20,000 hours or greater; see 40 CFR 372.3);and
      • The facility is included in Standard Industrial Classification (SIC) Codes 10 (except 1011, 1081, and 1094), 12 (except 1241), 20–39, 4911 (limited to facilities that combust coal and/or oil for the purpose of generating electricity for distribution in commerce), 4931 (limited to facilities that combust coal and/or oil for the purpose of generating electricity for distribution in commerce), 4939 (limited to facilities that combust coal and/or oil for the purpose of generating electricity for distribution in commerce), 4953 (limited to facilities regulated under RCRA Subtitle C, 42 U.S.C. section 6921 et seq.), 5169, 5171, and 7389 (limited to facilities primarily engaged in solvents recovery services on a contract or fee basis); and
      • The facility manufactures (defined to include importing), processes, or otherwise uses any EPCRA section 313 chemical in quantities greater than the established threshold in the course of a calendar year. Executive Order 13148 extended these reporting requirements to federal facilities, regardless of their SIC code.

  • Emergency and Hazardous Chemical Inventory (Tier II)

    1. Description: Submission of the Tier Two form (when requested) is required by Title III of the Superfund Amendments and Reauthorization Act of 1986,Section 312, Public Law 99-499, codified at 42 U.S.C. Section 11022. The purpose of this Tier Two form is to provide State and local officials and the public with specific information on hazardous chemicals present at your facility during the past year.
    2. Who must complete? The owner or operator of a facility must submit their Tier Two form if requested to do so by a State emergency response commission, a local emergency planning committee, or a fire department with jurisdiction over the facility.

    Hazardous Waste
    The Notification of Regulated Waste Activity instructions will assist you in obtaining an EPA Identification Number by completing and submitting the RCRA Subtitle C Site Identification Form (Site ID Form) [EPA Form 8700-12] for initial notifications; the instructions will also assist you in revising your Site ID Form information if you are required to submit a subsequent notification.
    This application is required for a Federal permit to treat, store, or dispose of hazardous waste under the Resource Conservation and Recovery Act (RCRA). It must be completed by anyone who owns or operates a facility where hazardous waste is treated, stored, or disposed (i.e. a TSDF).
    This form, or a state-specific form, must accompany all shipments of hazardous waste. It must be completed by any generator of hazardous waste shipping or transporting hazardous waste offsite.

    Storage Tanks
  • UST Notification Form
      UST owners must notify their designated state or local agency or department in two instances.

    • Within 30 days of bringing an underground storage tank (UST) into use, owners must submit the Notification for Underground Storage Tanks form (EPA Form 7530-1; Rev 6-2015) to their implementing agency.
    • Within 30 days of assuming ownership of a regulated UST, owners must submit the Notification of Ownership Change for Underground Storage Tanks form (EPA Form 6200-10) to their implementing agency.

    Stormwater
    EPA requires the following 2015 MSGP forms be submitted electronically unless granted a waiver from the EPA Regional Office: Notice of Intent (NOI), Change NOI, Annual Report, Notice of Termination (NOT), Discharge Monitoring Report (DMR), and Conditional No Exposure Certification (NOE).
    EPA has created the eNOI for construction sites, industrial facilities, pesticides, and vessel operators to apply for coverage under EPA's: Construction General Permit (CGP), Multi-Sector General Permit (MSGP), Pesticides General Permit (PGP), and Vessel General Permit (VGP).
    Form used by NPDES stormwater permittees to submit analytical monitoring results to the appropriate NPDES permitting authority.
    Form 2F must be completed by operators of facilities which discharge storm water associated with industrial activity or by operators of storm water discharges that EPA is evaluating for designation as a significant contributor of pollutants to waters of the United States, or as contributing to a violation of a water quality standard.

    Toxic Chemicals
    PCB transformer owners must register their transformer(s) with EPA using form 7720-12. This form cannot be filled out or submitted online.
    Form 7710-53 must be submitted before PCB activities are started, and should be re-submitted if any information in the form changes.

    Underground Injection Control
    This form must be completed by all owners or operators of Class I, II, and III injection wells and others who may be directed to apply for permit by the Director.
    Used when ownership of a UIC well is changed.
    Reports information such as injection pressure, injection rate, annular pressure, injection volume, temperature, and pH.
    Used to identify well location, well activity, well status, and the type of permit in place. Attachments to this form include: geologic information, well design and construction, a description of surface equipment, monitoring systems, and logging and testing results.
    Used to identify well location, type of well permit, anticipated daily injection volumes, injection interval, type of injection fluid, date drilling started and ended, casing and tubing information, and types of wire line logs.
    Annual report detailing injection pressures, total volume injected, and tubing-casing annulus pressure.
    Used to identify information such as total depth before and after rework and well casing information before and after rework.
    Used to detail the planned approach for plugging and abandoning a UIC well.
    Used to identify UIC wells and their operational status.
    Form which is required to be submitted prior to closing a Class V UIC well.

    Wastewater
    General information application. Must be submitted by all facilities applying for an individual NPDES permit, with the exception of POTWs, TWTDS, and MS4s serving a population greater than 100,000.
    Application for permit to discharge municipal wastewater from new and existing publicly owned treatment works (POTWs).
    Application for permit to discharge wastewater from concentrated animal feeding operations (CAFOs) and aquatic animal production facilities.
    Application for permit to discharge wastewater - existing industrial facilities (including manufacturing, commercial, mining and silvicultural operations) (Revisions proposed for Form 2C have been discontinued.)
    Application for permit to discharge process wastewater - for new industrial facilities.
    Application for facilities which do not discharge process wastewaters.
