Use the instructions for the Form R to complete the chemical specific information on pages 2 through 5 (Page 2, Page 3, Page 4of the Form R for each reportable toxic chemical. These are the chemicals for which a Page 3 of the Form R was begun at Step 29.

The DOE Site Office or Operations Office shall sign and submit the comprehensive Form R Reports for each DOE Site under its direction to EPA, the appropriate state, and EH-1.
